Claim Notification Deadline
You must give Provident written notice of claim within 30 days of the date disability
starts. If that is not possible, you must notify Provident as soon as you can.
When Provident receives your written notice of claim, it will send your claim forms.
If you do not receive the forms within 15 days after you sent the notice, you can send
written proof of claim without waiting for the form.
Proof of Claim Deadline
You must give Provident proof of claim no later than 90 days after the end of the
elimination period.
If it is not possible for you to give proof within these time limits, it must be given
as soon as reasonably possible. But you may not give proof later than one year after the
time it is otherwise required.
You must give Provident proof of continued disability and regular attendance of a physician
within 30 days of the date it requests the proof.
The proof must state:
- The date disability started;
- The cause of disability; and
- How serious the disability is.
Timing of Claim Payment
When Provident receives proof of claim, benefits payable under the policy will be
paid monthly during any period for which it is liable.
Benefit Recipient
All benefits are payable to you. But if a benefit is payable to your estate, or if
you are a minor, or you are not competent, Provident has the right to pay up to $1,000
to any of your relatives whom if considers entitled. If Provident pays benefits in
good faith to a relative, it will not have to pay such benefits again.
Conversion
Your group Long-Term Disability benefit is fully portable when you have completed
your studies at UCLA. You may continue your benefit by converting to an individual
disability income policy without providing a statement of good health. The cost of
this policy will be at your expense and based on your age at the time of conversion.
