Survivor Benefit
If you die
If you die, Provident will pay a benefit to your eligible survivor when it
receives proof that you died:
After disability had continued for 180 or more consecutive days; and
While receiving a monthly benefit.
The benefit will be an amount equal to three times your gross monthly benefit.
If payment becomes due to your children, payment will be made to:
Your children; or
A person named by Provident to receive payments on your children's behalf.
This payment will be valid and effective against all claims by others representing
or claiming to represent your children.
What the Plan Does Not Cover
Disabilities that are caused by the following are not covered:
War, declared or undeclared, or any act of war
Intentionally self-inflicted injuries
Active participation in a riot
Caused by, contributed to by, or resulting from a pre-existing condition;
and which begins in the first 12 months after your effective date
