Fee The application fee is $493.00 and the discounted licensing fee is $403.00, A check for $505 may be submitted with the application: the licensing fee of $403.00 may be sent to the Board at the time the license is to be issued. To qualify you for the licensing fee discount, the Director of Graduate Medical Education (Associate Dean Neil H. Parker, M.D.) must sign the L4 form. Without this form, the licensing fee is $806.00.
