Physicians practicing in a federal or state facility who are
required to obtain an individual DEA registration in order to carry out their duties,
are exempt from the $390 DEA registration fee (Section 1301.21 of Title 21, Exemption
from fees). You must use UCLA as your address, and your program director or department
chair must sign your application as the "Certifying Official." See instructions below.
- ADDRESS
For those house staff who receive mail in the UCLA Medical Center House Staff Mail
Room, use Room 16-155 CHS as your campus address. All others should use your
department address. The address for UCLA Medical Center is 10833 LeConte Avenue,
Los Angeles, CA 90095.
- REGISTRATION CLASSIFICATION
Mark the box indicating "Practitioner" and add "M.D." or "D.D.S." as appropriate.
- DO NOT CHECK THE BOX FOR ORDER FORMS.
This is NOT for presciptions.
- CERTIFICATION FOR FEE EXEMPTION
To be completed by your program director or department chair.
- It is recommended that you include a copy of your wallet-sized California medical
license with your application.
- Mail your application directly to the DEA. Their address is on the application. Keep a
copy for your records.
- The DEA registration takes 4-12 weeks to process. The phone number for the
local DEA office in Los Angeles is 213-621-6960.

Follow these steps:
- Click on "interactive form"
- Click on "go directly to form (PDF)"
- Click on the appropriate areas in the form and fill out your info.
- Under "registration classification", click on "practicioner" and type "MD"
- Under "drug schedules" click on everything.
- DO NOT PAY THE $390! You don't have to pay this fee and if you do pay it, you won't get a refund.
- Fill out the part about "certification for fee exemption".
