The purpose of this letter is to reiterate and emphasize the School’s and Program’s position regarding your conduct on _________.

It is the policy of the Geffen School of Medicine at UCLA that all trainees, patients and clinicians, faculty and employees shall be treated courteously, respectfully and with dignity. Your behavior on __________________was inappropriate and unacceptable. Such conduct will not be tolerated.

Your continued appointment to the Geffen School of Medicine at UCLA and the training program is contingent upon your conducting yourself professionally and courteously. Any further incidents of disruptive conduct will result in the initiation of necessary formal disciplinary action pursuant to the House staff Policies and Procedures.
